Each one on its own is easy. Together they are the product.
Pick a condition. You get what it means, where the market actually lands on it, and what we do instead. We argue these in public because a client should be able to check our homework.
Ask an AI engine the same question twice and you often get two different lists of names. A published study of nearly three thousand runs found it was less than a one-in-a-hundred chance that the same brand list came back twice. So a screenshot of one good answer proves nothing, and a screenshot of one bad answer proves nothing either.
Sells position tracking, as though being third in an AI answer were a rank you could hold. Some tools check once a week and draw a trend line through noise.
Repeated runs on a frozen question set, reported as a named rate per engine with the run count attached. When a number does not move, the report says so before you have to ask.
Local answers change block by block. A practice that gets named in one metro is invisible two hundred miles away, and the same question asked from two neighborhoods can return different names. A national average for a practice that serves one county is a number with no meaning in it.
The enterprise dashboards are geo-blind at the city level, because they were built for brands that sell everywhere. The local tools do vary location and then treat one run as a fact.
Probe from your market, the way your clients actually ask, and hold the location constant so the months compare. Geography and repetition together, not one or the other.
These are the fields where a wrong answer costs someone real money or real health, and the engines treat them accordingly: they hedge more, they lean harder on credentialed sources, and they weigh the named professional as much as the business. They are also the fields where your advertising is regulated, which rules out most of what marketing agencies do by reflex.
Serves everyone, which means the playbook is written for e-commerce and then handed to a surgeon. Testimonials, superlatives, and claims your board would object to arrive with it.
Publish only what is factual and verifiable, because that is what boards and bars allow and what the engines trust. Thirty-three specialty playbooks, each written from that field's own patient or client questions.
Before an engine names you, it reads what the web says about you in several places and looks for agreement. When your name, address, services, and people match everywhere, you read as a settled fact. When they conflict, the engine hedges, and a hedging engine names somebody else. The directory layer decides a surprising share of local answers.
Counts listings instead of checking them. Fifty directory submissions is an easy invoice line. Agreement across the fifteen that engines actually read is the harder, quieter job.
Build one canonical record, then make every surface the engines read agree with it, including the named practitioners. That is the Foundation, and it is a fixed-fee project, not a retainer.
A visibility score with no run count, no question list, and no formula behind it is a number you cannot argue with, which is exactly why it gets sold that way. We publish how we probe, how many runs stand behind a rate, what counts as mentioned versus cited, and where the method is weak.
Ships a dashboard with a proprietary index and a confident chart. The method behind the number stays a black box, which is a strange way to run a measurement business.

Three kinds of vendor, each good at something.
None of these are frauds, and we are not going to name them and pretend otherwise. They were built for different customers, and it shows the moment a two-physician practice walks in.
Breadth and engineering. Many engines, prompt-volume data, crawler analytics, and the budget to keep up as models change weekly. If you are a national brand, this is a real product.
They measure at the national level because that is who pays them, so a local answer never surfaces. They are also a dashboard: nobody does the work that moves the number. And they are priced monthly, in the thousands, before anyone touches your site.
Geography. They understood before anyone else that a local result is a map, not a list, and their grid views are genuinely useful for classic local search.
The statistics have not caught up to the medium. One run per point, treated as a fact, and a position reported as though AI answers had stable positions. Applied to AI, that method produces confident charts of noise.
Volume and speed. They will produce a great many properties with your name on them quickly, and the invoice is easy to understand.
It is a different business than ours. Paid placement in a listicle is an ad, and engines increasingly read it as one. In regulated fields it can also put you crosswise with your board or bar, which is a bigger bill than the campaign.
The gap is not that any of them is bad at their job. It is that measurement honesty, local geography, entity work, and regulated-field discipline have never been sold in one place, to a practice, at a price on the page.

These are not negotiable for a big enough contract. They are on every money page on this site for the same reason.
Nobody controls which sources an AI engine picks. A vendor promising you a citation is either guessing or lying, and in this field the guarantee is the single clearest mark of someone to walk away from.
We will not buy your way into a listicle or a link network. Paid placement is advertising, engines are getting better at reading it as advertising, and in regulated fields it can put your license in the conversation.
Nobody outside a model lab can put your practice into a model's training set, and anybody selling that is selling a thing that does not exist. What we influence is what the engines retrieve when someone asks.
No countdowns, no seats-remaining counters, no expiring quotes. The one exclusivity we do run is real and stated plainly: at Practice Plus and above we take one client per specialty per market, because we cannot honestly work for both sides of a street.
Management is month to month. Projects are fixed fee and finished when they are finished. If we stop earning it, stop paying it, and the pressure to keep earning it stays where it belongs.
